Group Membership Application

There are THREE parts to the group application process.

PART 1: Please fill out the form below to sign up and pay for membership as a group. Discounts tiers are automatically applied at checkout. Below are the discount tiers:

• 2-5 Members get 5% Discount in Membership
• 6-10 Members get 10% Discount in Membership
• 11-15 members get 15% Discount in Membership
• Over 15 Members get 20% Discount in Membership

PART 2: Once Part 1 is completed, your primary group contact person will need to assemble the following documents and email them to OEIS for inclusion in your application package: A current Curriculum Vitae for each provider (files should be provided in .pdf, .doc, or .docx format). Please email your supplemental application documents to [email protected] and you are encouraged to do so immediately after signup.

PART 3: Once that supplemental documentation is received, an OEIS representative will call the individual you have identified as your primary group contact to collect some additional information about your practice, as well as to provide instructions as needed and answer any questions you may have.

Did you know that you can merge your existing OEIS members into the new group?

You can, and here’s how it works. Add your existing active member(s) along with your new physicians to the group below.  When you advance to the payment part, the system looks up every physician’s e-mail address you entered to see if any currently exist in the system as OEIS members.  If one or more of your physicians is already an active member, the discounted difference between any existing member’s original expiration date and the newly formed group’s expiration date will simply be added to the order total when you check out.  The system will then advance any existing member’s expiration dates to match the new group as a whole, which will now be one year out from the group signup date.

IMPORTANT: Please ensure that if you are adding any existing OEIS members to your group that you use the same email addresses we have for them in the system.  Otherwise, the prorated discount remaining on the existing memberships will not be applied.  If you have any questions or concerns about this, feel free to reach us at [email protected] prior to completing the form below.
