There are several ways to become an OEIS Member

PATH 1 – Membership will be considered for physicians who perform procedures in outpatient endovascular or interventional laboratories. Candidates shall have certification by both a primary specialty board and an applicable subspecialty board which holds membership in the American Board of Medical Specialties or the Bureau of Osteopathic Specialists of the American Osteopathic Association and which have included endovascular procedures as an integral component of their training; including the fields of Vascular Surgery, Interventional Cardiology, and Interventional Radiology. Candidates should be performing endovascular and interventional procedures in an outpatient center. Lacking Board certification, candidates are expected to have made important clinical contributions over a period of years in an applicable field and provide evidence that he/she is recognized by peers in his/her community that he/she is a specialist.

PATH 2 – Membership will be considered for physicians who: (1) belong to practices which offer outpatient endovascular or interventional procedures and (2) have at least one interventionist in their practice who is an active member.

PATH 3 – Physicians who do not clearly meet the above-stated criteria for Pathways 1 and 2 will be reviewed for membership by the Membership Committee on a case-by-case basis for possible qualification and approval by the Board. For avoidance of doubt, only Active Members may vote on actions required by these Bylaws to be voted on or approved by members.

ALL THREE PATHS to Active Membership must also be supported by one of the following two methods:

A) ONE active member of OEIS must fill out a form affirming their support for your active membership, or

B) TWO physician colleagues who are not members of OEIS or your medical practice must fill out a Peer Survey Form on your behalf.

NOTE: OEIS will send your supporter(s) an email with a link to the proper support form when you submit your application. As the APPLICANT, it is your responsibility to contact your reference(s) and verify that their support forms were both received and completed on your behalf in a timely manner (within 2 weeks). Please ensure that the email addresses of your supporter(s) are entered correctly on the application!

Supplemental materials you’ll need to assemble PRIOR to submitting the application (ALL THREE PATHS):

• A copy of your current Curriculum Vitae for upload (file is required to be in .pdf, .doc, or .docx format)

Associate Membership will be considered for physicians who are interested in outpatient endovascular and interventional procedures but who do not currently perform such procedures, belong to practices which offer such procedures or meet the qualifications required of Active Members by Section 1.

Associate Membership will also be considered for non-physician individuals who are employed by or affiliated with corporations involved in providing outpatient endovascular and interventional procedures, or in providing goods and services for office- based cardiovascular procedures.

1. A copy of your current Curriculum Vitae

2. A brief statement of your involvement with outpatient endovascular and interventional procedures (text area provided)

A practice whose physicians ALL meet the qualifications for Active Membership can apply as a group. This brings a number of benefits:

1. A discount on annual membership dues

2. A simplified application and renewal process

3. A single application for the group, submitted along with the Curriculum Vitae of each physician

4. One OEIS member sponsor or 2 peer references for the group

5. The ability to add new members as they join the practice with pro-rated discounts

6. Easy annual renewal with a single payment, that can be handled by the practice administrator

Corporate Membership will be considered for corporations who support the objectives and activities of the Society.

1. A copy of your current Curriculum Vitae

2. A statement indicating how your company meets this requirement (text area provided)

Administrative & Affiliate Staff Membership is specifically for administrative and clinical staff affiliated with a current, existing OEIS member and/or participating registry site. If there is not a current OEIS member or participating registry site, please ensure that your physician and/or registry site is properly enrolled PRIOR to submitting this application.

This category of membership is specifically for resident, student, and fellow trainees affiliated accredited educational institutions.
